Please complete the following so we may present it to the manufacturer and act on your behalf in an attempt to gain their approval for an exchange under the terms of their warranty. You may find it helpful to reference the warranty card that came with your mattress.
After we receive this completed form from you, our Customer Care team will review and see if it is necessary to send a third party representative to your home to inspect. This representative will then determine if your complaint is covered under the manufacturer’s warranty.
*Please note we are unable to assist with warranty claims for products purchased at other retailers. These must be handled either through the original retailer or the manufacture directly.
